Job summary

Guardian Angel Hospice in Indiana seeks a compassionate Hospice Aide to support patients and families in their homes and facilities. You will provide direct patient care under the supervision of the RN Case Manager, assist with bathing, grooming, ADLs, and comfort measures aligned with each Plan of Care.

Responsibilities include monitoring patient condition, reporting changes to the RN and care team, assisting with transfers using adaptive equipment, and offering emotional support to patients

Qualifications

• Current CNA (Certified Nursing Assistant) or HHA (Home Health Aide) certification in the state of Indiana.

• Support patients with activities of daily living (ADLs) in a way that promotes dignity, comfort, and safety.
• Report observations and changes in patients’ physical or emotional condition to the RN Case Manager and interdisciplinary team.
• Respect professional boundaries while providing care in homes and healthcare facilities.
• Safely assist with transfers and mobility, including the use of hoyer lifts, gait belts, wheelchairs, walkers, canes, and crutches.
• Offer emotional support and compassionate presence to patients and caregivers.

Reliable transportation and a valid driver’s license.

Flexibility to participate in the on-call rotation schedule.

Physical ability to perform the duties of the role.

Strong time management skills and the ability to work independently while being part of a supportive team.
