Job summary

Choice Hospice, Inc. seeks a dedicated Hospice Aide (CNA/CPR) to join the North Houston Hospice Branch in Houston, TX. You will provide direct patient care under RN supervision, assist with transfers, and help create a safe, compassionate home care environment.

Responsibilities include reporting changes, maintaining infection control, and supporting patients and families with dignity and respect. Required: CNA/HHA, CPR, driver’s license, LTC, and preferred HomeCare HomeBase experience.

Qualifications

• Current CNA/HHA & LTC certifications are required.
• Valid Driver's License is required.
• Valid Auto Insurance is required.
• CPR Certification is required.
• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care to patients under direction of the RN and according to the Care Plan.
• Assist with safe transfers and ambulation as per policy.
• Report changes and document care to ensure continuity of care.
• Maintain infection control; provide a clean, safe environment.
• Promote respectful communication with patients and families.
• Support privacy and property of patients.
• Assist with nutrition and fluids as needed.
• Contribute to agency operations and time management.
• Perform treatments and procedures per policy.
