Job summary

Choice Health At Home is seeking a Hospice Aide (CNA/CPR) to provide direct patient care under RN supervision in the patient’s home. Responsibilities include safe transfers, documentation, infection control, and maintaining a respectful, clean environment while supporting nutrition and hydration.

The role requires CNA/HHA and LTC certifications, a valid driver’s license and auto insurance, CPR, and preferred HomeCare HomeBase experience. It is a full-time position in Montgomery, TX.

Qualifications

• Current CNA/HHA & LTC certifications.
• Valid Driver’s License.
• Valid Auto Insurance.
• CPR Certification.
• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care under direction of the RN and according to the Aid Plan of Care.
• Assist with safe transfers and ambulation per agency policy.
• Report changes and document care rendered to patients to ensure continuity of care.
• Practice infection control principles to provide a clean, safe environment.
• Promote respectful communication with patient/family and other employees.
• Promote privacy and property respect for patients.
• Assist with proper nutrition and adequate fluid intake.
• Contribute to efficient operation and time management of the agency.
• Perform treatments and procedures per agency policy.
• Promote the agency philosophy and administrative policies to ensure quality of care.
