Job summary

Choice Health at Home is seeking a Hopestone Hospice Aide- CNA/CPR to join our Missouri City-based team. This full-time role provides direct patient care under RN supervision in patients' homes and works to maintain safety, infection control, and comfort for clients and families.

Qualifications include CNA/HHA & LTC certifications, CPR, a valid driver’s license, and insurance. HomeCare HomeBase experience is a plus. We offer comprehensive benefits and a supportive care environment.

Qualifications

• Current CNA/HHA & LTC certifications.
• Valid Driver’s License.
• Valid Auto Insurance.
• CPR Certification.
• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care to patients under direction of the RN and according to the Aid Plan of Care.
• Assist with safe transfers and ambulation per agency policy.
• Report changes and document care rendered to ensure continuity of care.
• Practice infection control principles and maintain a clean, safe environment.
• Promote respectful communication with patient/family and staff.
• Respect patient privacy and property.
• Assist with proper nutrition and fluid intake.
• Demonstrate time management and support efficient agency operation.
• Perform treatments and procedures per agency policy.
• Support the agency philosophy and policies to ensure quality of care.
