Job summary

Choice Health At Home in Austin, TX is seeking a Hospice Aide (CNA) to provide compassionate home-based care under RN supervision. The role emphasizes safe transfers, documentation, infection control, and respectful communication with patients and families.

Full-time opportunity at the Austin Hospice Branch. Requires CNA/HHA and LTC certifications, CPR, valid driver’s license, and auto insurance; HomeCare HomeBase experience preferred. Includes a 401k plan after probationary period.

Qualifications

• Current CNA/HHA and LTC certifications required.
• Valid driver’s license and auto insurance.
• CPR certification required.
• Experience with HomeCare HomeBase preferred.

Responsibilities

• Provide direct patient care to patients under direction of the RN and according to the Aid Plan of Care.
• Assist with safe transfers and ambulation per agency policy.
• Report changes and document pertinent information to ensure continuity of care.
• Maintain infection control; provide a clean, safe environment.
• Promote respectful communication with patient/family and staff.
• Respect patient privacy and property at all times.
• Assist with nutrition and fluid intake as needed.
• Demonstrate time management in agency operations.
• Perform treatments and procedures per agency policy.
• Support the agency philosophy and policies to ensure quality of care.
