Job summary

St. Croix Hospice in Wisconsin is seeking a Hospice Aide (CNA) to join our interdisciplinary care team. You will provide compassionate, hands-on personal care to patients in their homes under the direction of a Registered Nurse, ensuring comfort and dignity at end of life.

Qualifications

• Current CNA certification in the state of practice.
• Valid driver’s license with clean driving record and auto insurance.
• Provide personal care and support daily living activities in patients' homes.
• Record vital signs and report observations to the care team.
• Travel to patient homes and document services accurately.

Responsibilities

• Provide personal care including bathing, skin/nail care, oral hygiene, dressing/undressing.
• Record vital signs and report observations to the care team.
• Assist with meals and feeding as needed.
• Promote patient comfort, dignity and safety.
• Travel to patient homes and provide care as scheduled.
• Maintain accurate documentation of services provided.

As a Hospice Aide (CNA), you’ll be an essential part of our interdisciplinary care team, delivering compassionate, hands‑on care to patients in their homes. Working under the direction of a Registered Nurse, you’ll support patients with personal care and daily living activities, helping ensure comfort, dignity, and the highest possible quality of life at the end of life.

Qualifications

Hospice Aide (CNA) Requirements & Responsibilities:

• Current CNA certification in the state of practice
• Valid driver’s license, clean driving record, and auto insurance

• Assist with meal planning, preparation, and feeding as needed
• Promote patient comfort, dignity, and safety
