Job summary

St. Croix Hospice is seeking a compassionate Hospice Aide (CNA) to work in patients' homes, under RN supervision, delivering hands-on personal care and support for daily living activities.

This full-time role involves 1:1 patient interaction, travel to multiple home visits, and accurate documentation of services provided. The position requires an Illinois CNA certification, valid driver's license, and reliability for home visits.

Qualifications

• Current Illinois CNA certification.
• Valid driver’s license with clean record and auto insurance.
• Comfort with in-home patient care and daily living activities.
• Ability to travel to patient homes throughout the day.

Responsibilities

• Provide personal care including bathing, skin and nail care, oral hygiene, dressing/undressing.
• Record vital signs and report observations to the care team.
• Assist with meal planning, preparation and feeding as needed.
• Promote patient comfort, dignity, and safety.
