Job summary

Heart to Heart Hospice is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under RN supervision in a hospice setting. You will assist with transfers and ambulation, report changes, and document care to ensure continuity of care.

The ideal candidate holds a current CNA license, upholds confidentiality, and demonstrates genuine care for ill and elderly patients. A driver's license and reliable transportation are required for this role.

Qualifications

• High School Diploma or GED preferred.
• Current unencumbered CNA license per Nurse Aide Registry.
• Minimum 1 year experience as Hospice Aide or Nursing Assistant in care settings.
• Demonstrates compassionate care and integrity in handling confidential information.
• EMR experience is a plus; valid driver's license and reliable transportation.

Responsibilities

• Provide direct patient care to patients under the direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Assist with safe transfers and ambulation per agency policy.
• Report and document changes and care provided to ensure continuity of care.
• Practice infection control principles.
• Communicate with patient/family and other employees in a respectful manner.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and policies.

Hospice Aide - CNA

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:
