Job summary

Heart to Heart Hospice is hiring a Hospice Aide (CNA) to provide direct patient care under the supervision of the registered nurse and per the Plan of Care. You will assist with transfers and ambulation and document care to ensure continuity.

The role requires a current CNA license and at least one year of experience in a hospice or similar setting, with a compassionate approach and reliable transportation.

Qualifications

• High school diploma or GED preferred
• Current CNA license required
• At least 1 year as Hospice Aide/Nursing Assistant in health care setting
• Demonstrates compassion and integrity in handling patient care
• Reliable transportation and valid auto liability insurance

Responsibilities

• Provide direct patient care under RN direction per Plan of Care
• Assist patient with safe transfers and ambulation
• Report and document care provided to ensure continuity of care
• Follow infection control principles
• Communicate respectfully with patient, family and staff
• Timely submission of all documentation
• Perform other duties as required
• Ensure compliance with laws, regulations, and policies

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
