Job summary

Heart to Heart Hospice is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under the supervision of the RN and within the Plan of Care. The role requires a current CNA license, high school diploma or GED, and at least one year of hospice or nursing assistant experience.

Responsibilities include assisting with transfers, reporting changes, documenting care, and maintaining infection control. A valid driver's license and reliable transportation are required.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA certification.
• Current CNA license required.
• At least one year as Hospice Aide or Nursing Assistant in a healthcare setting.
• Demonstrates care for ill and elderly with integrity and confidentiality.
• EMR experience is a plus.
• Must have valid driver's license and reliable transportation.

Responsibilities

• Provide direct patient care under RN direction and Plan of Care.
• Assist patient with safe transfers and ambulation per policy.
• Report and document changes and care provided to ensure continuity of care.
• Follow infection control principles.
• Communicate with patient/family and staff respectfully.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with laws, regulations and policies.

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
