Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to provide compassionate direct patient care under RN supervision, following the established care plan. You will assist with transfers, report changes, and document care to ensure continuity.

The role requires a current CNA license, high school diploma or GED, and reliable transportation. EMR experience is a plus, with company-provided benefits for full-time employees.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist patients with safe transfers and ambulation.
• Report and document changes and care provided to ensure continuity of care.
• Follow infection control practices and safety policies.
• Communicate with patients, families, and other staff respectfully.
• Submit all required documentation timely.
• Perform other duties as assigned.
• Comply with local, state, and federal regulations and agency policies.

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
