Job summary

St. Croix Hospice in Jeffersonville, IN is seeking an Entry Hospice Aide to join our family.

You will work under RN supervision, provide direct patient care, document care accurately, and help maintain safety and quality in daily routines. You will perform bathing, mobility, meals, vital signs, and reporting changes to the RN Case Manager while delivering compassionate, confidential care to patients and families in a 1:1 setting.

Responsibilities

• Provide patient care including bathing, nail/skin care, dressing/undressing, dental and other personal hygiene while maintaining a sanitary and orderly environment.
• Offer positive, respectful, and confidential patient/family communication.
• Take and record vital signs, and assist with self-administered medications.
• Plan and prepare nutritious meals, including shopping and feeding the patient as needed.
• Assess and report patient's condition and changes to the RN Case Manager.
• Assist in ambulation, exercise, transfers, positioning and support a complete range of motion and other therapy activities.

As the Hospice Aide, you are a paraprofessional and valued member of the interdisciplinary care team. You will work under RN supervision and focus on quality, continuity, and safe patient care.In this role you will be responsible for timely and accurate documentation of care.

QUALIFICATIONS

• At least 18 years of age
• Active Certified Nursing Assistant (CNA) designation current with state-specific requirements
