Job summary

St. Croix Hospice in Jeffersonville, IN is seeking an Entry-level Hospice Aide to support RN-supervised care. You will document care timely and accurately, assisting with daily living activities and ensuring patient comfort.

Responsibilities include bathing, vital signs, meal preparation, and reporting changes to the RN Case Manager, while maintaining HIPAA privacy and compassionate communication with patients and families.

Qualifications

• Must be at least 18 years old.
• Active CNA designation current with state requirements.
• Licensed and insured driver with a reliable vehicle.
• Commitment to privacy and HIPAA standards.

Responsibilities

• Provide patient care including bathing, grooming, and personal hygiene while maintaining a sanitary environment.
• Communicate with patients and families in a respectful, confidential manner.
• Record vital signs and assist with self-administered medications.
• Plan and prepare nutritious meals and assist with feeding as needed.
• Assess patient condition and report changes to the RN Case Manager.
• Assist with mobility, transfers, and range-of-motion therapies.

As the Hospice Aide, you are a paraprofessional and valued member of the interdisciplinary care team. You will work under RN supervision and focus on quality, continuity, and safe patient care. In this role you will be responsible for timely and accurate documentation of care.

JOB RESPONSIBILITIES

• You will provide patient care including bathing, nail/skin care, dressing/undressing, dental and other personal hygiene while maintaining a sanitary and orderly environment
• You will offer positive, respectful, and confidential patient/family communication
• You will be responsible for taking and recording vital signs, and assisting with self-administered medications
• You will plan and prepare nutritious meals, including shopping and feeding the patient as needed
• You will assess and report patient's condition and changes to the RN Case Manager
• You will assist in ambulation, exercise, transfers, positioning and support a complete range of motion and other therapy activities
