Job summary

MJHS seeks an experienced Registered Nurse to join our hospice and palliative care team in Newark, NJ. The role covers on-call visits after hours, coordinating with the triage center and On-Call Manager to address urgent physical and psychosocial needs and ensure a safe plan of care.

The ideal candidate has 2+ years in Medical-Surgical, Hospice or Oncology nursing, NYS RN licensure preferred, and CHPN certification within one year. Proficiency with Word, Excel, and Windows is expected.

Qualifications

• Graduate from an accredited School of Nursing.
• At least two years experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing preferred.
• New York State RN licensure preferred; CHPN certification within one year of hire.
• Proficient with Word, Excel and Windows applications.

Responsibilities

• Provide after-hours on-call patient visits, scheduled or unscheduled, for urgent physical and psycho-social issues.
• Coordinate with Call Center Triage and On-Call Manager; respond via cell phone as needed.
• Educate patients and primary caregivers on changes to plan of care, including medication management, safety and symptom management.
• Attend patient deaths to provide emotional support and guidance to family; report changes to triage and hospice team.
• Follow NYS Standards for Professional Registered Nurse.
