Job summary

Elara Caring is seeking an After-Hours/On Call Registered Nurse to support hospice patients and families with comprehensive nursing care. You will assess needs, monitor changes, and collaborate with the care team to ensure timely, compassionate service.

The role requires active RN licensure, a nursing degree, and at least one year of clinical nursing experience. Travel between patient homes is expected, with education and bereavement support responsibilities included.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, AS or BS).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.
• Ability to lift up to 50 pounds and travel between patient homes.

• Monitor patient condition and communicate changes to physician and care team.
• Educate and support patients, families, caregivers, and facility staff in the Plan of Care.

• Provide compassionate support at time of death and complete bereavement assessments.

As an After-Hours/On Call Registered Nurse, you’ll contribute to our success in the following ways:

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
