Job summary

Elara Caring is seeking a Hospice After-Hours Registered Nurse to support patients and families in home and facility settings. You will assess needs, monitor conditions, and communicate changes to the care team, ensuring timely, compassionate care.

Responsibilities include educating staff and families on the Plan of Care, hospice philosophy, pain and symptom management, and bereavement support. An RN license and nursing degree are required, with 1+ year of clinical experience; home health

Are you one of them? As an Hospice After-Hours Registered Nurse, you’ll contribute to our success in the following ways:

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred.
