Elara Caring is seeking an After-Hours/On Call Registered Nurse for a 7-on/7-off schedule, Wed-Wed, 5P-8A. The role focuses on providing hospice care, assessing patients and families, educating care teams, and ensuring timely communication with physicians.

The ideal candidate is an RN with 1+ year of clinical nursing, home health experience preferred, and would travel between patient homes to deliver compassionate, high-quality care.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Bachelor's).
• 1+ year of clinical nursing experience.
• Home Health experience preferred.
• Ability to lift up to 50 pounds and assist with patient handling.
• Valid driver’s license or reliable transportation for travel to patient homes.

Job Description

7 ON 7 OFF POSITION WED-WED 5P-8A To continue to be an industry pioneer in delivering unparalleled care, we need an After-Hours/On Call Registered Nurse with commitment and compassion.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

Requirements

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred.
