Job summary

Residential Home Health and Residential Hospice is seeking an After Hours Licensed Practical Nurse to provide end-of-life care to patients in their homes in the Cadillac area. The schedule is Monday through Friday, 4pm-12am, enabling autonomy and meaningful patient relationships while collaborating with an interdisciplinary team.

Hearing a strong emphasis on compassionate care, thorough EMR documentation, and ongoing patient and family education during evenings and weekends.

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work.
• Two years of LPN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred.
• Current driver's license and ability to spend ~20% of your day driving to/from patient locations.

• Ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
