Job summary

Residential Home Health and Hospice in Cadillac is seeking an After Hours Licensed Practical Nurse to provide end-of-life care in patients' homes. This role supports an interdisciplinary team, conducts ongoing patient assessments, and documents in EMR while traveling to patient locations within the territory.

The position requires a current LPN license, two years of LPN experience, a valid driver’s license, and the ability to drive about 20% of the time. A hospice background is preferred.

Qualifications

• Active LPN license in the state you work.
• Two years of LPN experience and the clinical expertise to deliver quality patient care; hospice experience preferred.
• Valid driver’s license and ability to spend ~20% of your day driving to patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting.
• Provide after-hours assessments focused on comfort and symptom management; escalate to RN as needed.
• Educate patients and caregivers on end-of-life plans and expectations.
• Ensure thorough, compliant EMR documentation.
