Job summary

Residential Home Health and Residential Hospice is seeking an experienced Hospice LPN to provide compassionate end-of-life care in patients' homes in Kansas City, MO and surrounding areas.

You will work with an interdisciplinary team, conducting assessments, educating families, and ensuring accurate EMR documentation while traveling to patient locations within your territory.

Qualifications

• Active LPN license in the state you work.
• Two years of LPN experience; hospice experience preferred.
• Valid driver’s license and ability to spend ~20% of your workday driving to patient locations.

LPN Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work
• Two years of LPN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
