Residential Home Health and Hospice in Cadillac is seeking an After Hours Licensed Practical Nurse to provide end-of-life care in patients' homes. This role supports an interdisciplinary team, conducts ongoing patient assessments, and documents in EMR while traveling to patient locations within the territory.

The position requires a current LPN license, two years of LPN experience, a valid driver’s license, and the ability to drive about 20% of the time. A hospice background is preferred.

We are hiring an After Hours Licensed Practical Nurse for a Hospice position providing end-of-life care to patients in their homes in the Cadillac area.

The schedule will be Monday through Friday, 4pm-12am.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide after-hours support by conducting ongoing patient assessments focused on comfort, symptom management, and changes in condition, escalating clinical concerns to the RN as appropriate
• Educate and support patients and caregivers during evenings, nights, and weekends by reinforcing end-of-life care plans, symptom relief strategies, and what to expect throughout the hospice journey
• Ensure thorough, compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

After Hours LPN Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work
• Two years of LPN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
