Residential Home Health and Hospice in Carlinville, IL is hiring a Licensed Practical Nurse to provide end-of-life care in patients' homes. You will work with an interdisciplinary team to deliver compassionate care and maintain thorough EMR documentation.

• Educate and guide patients and caregivers on end-of-life care, symptom relief strategies, and what to expect throughout the hospice journey
• Ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

LPN Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work
