Job summary

Luminos Hospice is hiring a Hospice Admit RN to lead admission assessments and coordinate the transition of patients into our care. You will partner with care teams to ensure families receive compassionate, organized admission experiences.

The role requires an active RN license, a valid driver’s license, and CPR certification. Two years of clinical RN experience with one year in hospice is preferred; travel expectations may apply within the region.

Qualifications

• Current State license as a Registered Nurse (RN).
• Current driver’s license.
• Current CPR Certification.

Responsibilities

• Coordinates admission processes for new hospice patients.
• Collaborates with care teams to ensure smooth communication.
• Educates families about patient condition, needs and care plan.
• Performs comprehensive head-to-toe assessments.
• Documents timely and accurate patient information.
• Other duties as assigned by supervisor.
