Job summary

Luminos Hospice in Missouri is seeking a dedicated Hospice Admit RN to coordinate patient admissions and deliver compassionate care coordination for our end-of-life patients and their families. You will conduct comprehensive assessments, oversee admission procedures, educate families, and collaborate with the care team to ensure seamless communication and thorough documentation.

A current RN license, driver's license, CPR certification, and two years of RN experience with hospice preferred are

Job Summary:

As an admission nurse, you'll conduct comprehensive patient assessments, facilitate seamless admission procedures, and deliver compassionate care coordination, ensuring the best support for hospice patients and their families.

Requirements

Licensure:

Current State license as a Registered Nurse.

Current driver’s license.

Current CPR Certification.

Experience:

Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice preferred.
