Job summary

Luminos Hospice is seeking a compassionate Hospice Admit RN to lead patient admissions and coordinate seamless transition into care. You will perform comprehensive assessments, collaborate with clinical teams, and educate families about conditions, needs, and care plans in a patient-centered environment.

The ideal candidate brings RN licensure, CPR certification, and 2+ years of nursing experience, with hospice exposure preferred.

Qualifications

• Current State license as a Registered Nurse.
• Current driver’s license.
• Current CPR Certification.
• Two+ years of RN clinical care experience; hospice preferred.

Responsibilities

• Coordinates and oversees all aspects of new patient admissions.
• Collaborates with patient care teams to ensure seamless communication.
• Educates families on patient condition, needs, and care.
• Performs head-to-toe physical assessments.
• Timely and accurately completes documentation.
• Other duties as assigned.
