Job summary

Luminos Hospice in Overland Park, KS seeks an experienced Hospice Admit RN to lead patient admissions, perform comprehensive assessments, and coordinate compassionate care from intake through admission.

This full-time role requires an active RN license, strong communication, attention to detail, and the ability to educate families; you will collaborate with clinical teams to ensure smooth transitions and accurate documentation.

Qualifications

• Current State license as a Registered Nurse.
• Current driver’s license may be required depending on territory.
• Two years RN experience in clinical care; one year hospice preferred.

Responsibilities

• Coordinates all aspects of new patient admissions.
• Collaborates with patient care teams for seamless communication.
• Educates families on patient condition, needs, and care plan.
• Performs head-to-toe physical assessments.
• Completes documentation timely and accurately.

Job Summary:

As an admission nurse, you'll conduct comprehensive patient assessments, facilitate seamless admission procedures, and deliver compassionate care coordination, ensuring the best support for hospice patients and their families.

Responsibilities:

• Collaborates with patient care teams to ensure seamless communication

• Timely and accurately completes documentation

Requirements

Licensure:

Current State license as a Registered Nurse.

Current driver’s license.

Current CPR Certification.

Experience:

Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice preferred.

Skills:

Nursing skills as defined as generally accepted standards of practice.

Knowledge of state and federal regulations.

Ability to work independently and make sound decisions.

Strong attention to detail and organizational habits.

Excellent verbal and written communication skills.

Good interpersonal skills.
