Job summary

Luminos Hospice is seeking an Assistant Director of Nursing to support the Director of Nursing in delivering compassionate hospice care. This role oversees clinical operations, supervises nursing staff, and ensures compliance with hospice regulations and agency policies, aiming for excellent patient outcomes.

The ADON will coordinate admissions, care plans, and interdisciplinary services while leading quality improvement, staff education, and regulatory adherence across the organization.

Qualifications

• Current LPN/RN license in the state of practice.
• Current Driver's License.
• Minimum of 2 years of nursing experience, preferably in hospice or palliative care.
• Excellent verbal and written communication skills.
• Ability to work independently and make sound decisions.
• Strong attention to detail and organizational habits.

Responsibilities

• Supports the Director of Nursing with daily clinical operations.
• Supervises and mentors nurses, aides, and other clinical staff.
• Coordinates patient admissions, care plans, and interdisciplinary services.
• Ensures patients receive safe, compassionate end-of-life care.
• Reviews documentation for accuracy, completeness, and regulatory compliance.
• Monitors quality measures, incidents, complaints, and patient outcomes.
• Helps manage staffing, scheduling, and on-call coverage.
• Educates staff on hospice policies, procedures, and clinical best practices.
• Communicates with physicians, families, pharmacies, facilities, and the hospice team.
• Assists with audits, surveys, performance improvement, and regulatory compliance.
• Provides direct patient care or on-call support when needed.
• Performs the Director of Nursing's duties when the DON is unavailable.
