Job summary

Luminos Hospice is seeking an experienced RN Case Manager to lead a care team and coordinate high-quality home-based hospice services. You will work with the Clinical Director to develop care plans, educate families, and ensure timely documentation while maintaining compliance and compassionate patient-centered care.

The role requires licensure as a Registered Nurse, a valid driver’s license, CPR certification, and at least two years of nursing experience, with hospice experience preferred.

Qualifications

• Current State license as a Registered Nurse.
• Current driver’s license.
• Current CPR Certification.
• Two (2) years' RN experience in a clinical setting, hospice preferred.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Administers and maintains catheters.
• Complete documentation timely and accurately.
• Supervises HHAs.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Other duties as assigned.

Requirements

Licensure:
Current State license as a Registered Nurse.

Experience:
Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice preferred.

Skills:
Nursing skills as defined as generally accepted standards of practice.

Knowledge of state and federal regulations.

Ability to work independently and make sound decisions.

Strong attention to detail and organizational habits.
