Hope Hospice in New Braunfels, TX, is seeking an Admissions and Transitional Care Nurse to join our compassionate team. This role coordinates initial assessments, plans care with an interdisciplinary team, and manages admissions and discharge planning.

The position requires weekend 12-hour shifts (Sat/Sun/Mon) and strong documentation skills. Applicants must hold a BSN with a current TX license and provide reliable transportation.

Qualifications

• Graduate of an approved nursing program.
• Current Texas nursing license.
• Reliable transportation and a valid Texas driver's license.
• One year of clinical nursing experience; case management experience preferred.
• Strong assessment and communication skills.

Responsabilités

• Receives referrals and conducts initial nursing assessments.
• Coordinates establishment of the plan of care with the interdisciplinary team.
• Schedules and provides admissions and discharge planning.
• Works weekend and 12-hour shifts with Monday GIP focus.
• Maintains accurate documentation and effective patient/family communication.
