Job summary

Hospice of South Texas (HOST) is seeking a compassionate RN Admission Nurse to complete hospice admissions and provide comprehensive skilled nursing care to patients in their place of residence. This role acts as a clinical liaison between patients, families, referral partners, and the interdisciplinary team to ensure timely admission, care coordination, and regulatory compliance.

The position requires graduate nursing education, Texas RN licensure, and valid driver’s license with reliable

Qualifications

Current Texas RN license in good standing.
Experience in home-based care or hospice preferred.
CPR certification and Hepatitis consent/declination.
Valid driver's license and reliable transportation.

Responsibilities

Perform comprehensive admission assessments addressing physical, functional, psychosocial, spiritual, and environmental needs.
Coordinate care with the interdisciplinary team (IDT), physicians, and referral partners.
Educate patients and families on disease progression, symptom management, nutrition, self-care, and end-of-life expectations.
Complete and submit all documentation timely per HOST policies and regulatory standards.
Participate in assigned on-call rotation providing patient/family support.
