Job summary

Valor Hospice is seeking a Registered Nurse to perform admissions and provide nursing care in patients' homes in the Houston area. You will coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agencies while managing documentation and on-call duties.

Qualified candidates have an RN license, 2 years of clinical RN experience, and at least 1 year in hospice. Strong interpersonal skills and reliable transportation are essential to excel in this role.

Qualifications

Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure: Current State license as a Registered Nurse; Current driver's license.
Experience: Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
Skills: Nursing skills per standard practice; knowledge of state and federal regulations.
Good interpersonal skills and reliable transportation.

Responsibilities

Assess hospice patients and identify physical, psycho-social, and environmental needs.
Develop and document the plan of care respecting assessments.
Provide nursing care with infection control measures.
Collaborate with the Interdisciplinary Team to meet patient/family needs.
Communicate with attending physicians and hospice physicians.
Submit required paperwork timely and maintain competency; participate in QAPI.
