Job summary
Hospice of South Texas seeks an RN Admission Nurse to complete hospice admissions and provide comprehensive skilled nursing care in patients' homes. This role acts as a clinical liaison among patients, families, referral partners, and the interdisciplinary team to ensure timely admission, care coordination, and regulatory compliance.
The ideal candidate demonstrates strong assessment, communication, and independent-working abilities, with a commitment to hospice philosophy and patient-centered
Qualifications
- Graduate of an accredited nursing program (Diploma/ADN/Bachelor of Science in Nursing).
- Current Texas RN license.
- Valid Texas Driver's License with reliable insured transportation.
- CPR certification and Hepatitis consent/declination.
- Strong interpersonal, communication, and assessment skills.
- Ability to work independently in home-based settings.
Responsibilities
- Conduct comprehensive admission assessments addressing physical, functional, psychosocial, spiritual, and environmental needs.
- Review referral and admission documentation for eligibility and appropriateness.
- Apply hospice admission criteria to determine level of care and eligibility.
- Assess caregiver readiness and support needs at admission.
- Establish individualized Plans of Care and revise as needed.
