Hope Hospice in Bulverde, TX is seeking an experienced RN Care Manager to deliver comprehensive patient care as part of the interdisciplinary team, supporting patients and families at home.

Required BSN and Texas RN license, 1+ year hospice or seriously ill patient care experience, reliable transportation, and strong communication. We offer flexible schedules and a competitive benefits package.

Qualifications

• RN licensure in Texas.
• BSN preferred.
• Experience in hospice or care of seriously ill patients.

Responsabilités

• Direct nursing care and coordination.
• Respond to patients' and families' hospice needs.
• Perform skilled nursing assessments and document changes.
• Coordinate with IDT and physician for care plans.
• Provide medications and treatments as ordered.
• Support caregiver education and bereavement processes.
