Job summary

VITAS Healthcare seeks an Admissions Coordinator to convert referrals into admissions through intake, communication, and use of clinical and administrative systems. You will work with patients, families, referral sources, and program staff to start care for VITAS patients.

The role involves processing referrals from multiple channels, following up on charts, ensuring data integrity, and verifying hospice benefits.

Qualifications

Two or more years related experience in medical terminology and/or in a call center environment.
Ability to attain goals in a fast-paced, dynamic environment.
Excellent written and verbal communication at all levels.
Strong computer literacy and ability to navigate automated systems and multiple software packages.

Responsibilities

Process new referrals via telephone, email, fax and website inquiries.
Follow up on multiple lists of pending patient charts daily.
Enter accurate information to support data integrity and maximize reimbursement.
Verify Medicare and Medicaid hospice benefits.
Notify Managed Care of commercial insurances and plans.
Notify the Marketing Reps of new referrals after hours.
Call patients and families to offer same-day admissions.
Assign appointments to Admissions Reps and provide reports after-hours.
Complete admission consent paperwork as needed.
Establish relationships with internal and external customers.
Provide training and mentoring to new team members.
Participate in morning staffing check-ins and adjust schedules as needed.
Support mission, core values, and Care Connection Center culture.
