Hospice of Central PA is seeking a Hospital Based Social Worker in Harrisburg to join the Admissions Department. This full-time role offers 80 hours per biweekly pay period and requires MSW; LSW or LCSW licensure must be obtained within 2 years of hire. The position emphasizes collaboration across hospitals and home care settings.

You will conduct assessments, plan patient-centered care, and support transitions of care with families and care teams. A comprehensive benefits package is included.

Qualifications

• MSW required.
• LSW or LCSW licensure must be obtained within 2 years of hire date.
• Two years’ experience in health care setting preferred.

Responsabilités

• Collaborates with hospice interdisciplinary team, hospitals, and care facilities to develop comfort-focused plans of care and conduct comprehensive assessments.
• Collaborates with patients, families, and case managers during hospital discharge planning for safe transitions of care.
• Performs joint home visits with admissions nurses when psychosocial complexity is present.
• Conducts informational and follow-up visits for patients in hospitals, facilities, and home settings.
• Supports initial response for urgent referrals needing immediate response.
