Hospice of Central PA seeks a hospital-based Social Worker in Admissions who will coordinate referrals, explain hospice benefits, and assist with discharge planning. The role requires MSW and may require LSW/LCSW within 2 years, with a preference for 2 years of healthcare experience. This is a PRN/Per Diem position with day shift hours.

The Social Worker will collaborate with the IDT, caregivers, and hospital staff to ensure smooth transitions and comprehensive psychosocial support.

Formación

• MSW required.
• LSW or LCSW is also required or must be obtained within 2 years of hire date.
• Two years’ experience in the health care setting preferred.

Responsabilidades

• Collaborates with hospital providers and staff IDT to develop a plan of care and evaluate SW interventions.
• Conducts admission assessments and follow-ups on hospitalized patients.
• Documents accurately and concisely at the time of service.
• Initiates and revises the plan of care with input from IDT, patient, and caregivers.
• Participates in hospital discharge planning and IDT meetings.
• Provides education and support to patients and caregivers regarding diagnosis, care, and end-of-life expectations.
