Job summary

Hospice & Community Care is seeking a Social Worker for Home Hospice to collaborate with the Interdisciplinary Team and provide psychosocial support to patients and families.

The role requires MSW and licensure within two years, covering Lancaster and surrounding counties, with an orientation period of 4–5 weeks. Per diem, day shift, 64 hours per two weeks, with community-based responsibilities.

Qualifications

• MSW required; LSW or LCSW must be obtained within two years from date of hire.
• Position covers Lancaster, York and surrounding counties.
• Must complete 4–5 weeks of full‑time day‑shift orientation.

Responsibilities

• Provide social work interventions, education, counseling and support services to address psychosocial needs of patients and caregivers.
• Collaborate with the Interdisciplinary Team to coordinate care and resources.
