Hospice & Community Care is looking for a dedicated Social Worker to join the Admissions department. This position primarily involves hospital-based work during the day, ensuring effective referral coordination while supporting patients and families. The ideal candidate will have an MSW and must obtain LSW or LCSW within two years of hire.

The Social Worker will assist the interdisciplinary team in planning care, conducting assessments, and providing education about hospice services. Two years of healthcare experience is preferred, making this role crucial in enhancing patient care at end-of-life.

Qualifications

• MSW degree is essential for the role.
• License as a Social Worker (LSW) or LCSW must be obtained within 2 years of hire.
• Two years of experience in a healthcare setting is preferred.
