Qualifications

• Master of Social Work (MSW) is required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) must be obtained within 2 years of hire.
• Two years' experience in a healthcare setting preferred.

Responsibilities

• Facilitates referral coordination with hospital staff and teams.
• Conducts admission assessments and follow-ups on hospitalized patients.
• Documents service provisions accurately.
• Supports hospital discharge planning and provides education to patients.
