Hospice & Community Care is seeking a Social Worker for the Admissions department, primarily based in hospitals during the day. The role involves coordinating patient admissions, providing psychosocial support, and facilitating effective discharge planning.

Qualified candidates will hold an MSW and have experience in healthcare settings. Responsibilities include collaborating with medical teams, conducting assessments, and educating patients and caregivers.

Qualifications

• Master of Social Work (MSW) is required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) must be obtained within 2 years of hire.
• Two years' experience in a healthcare setting preferred.

Responsabilités

• Facilitates referral coordination with hospital staff and teams.
• Conducts admission assessments and follow-ups on hospitalized patients.
• Documents service provisions accurately.
• Supports hospital discharge planning and provides education to patients.
