Vitality Hospice is seeking an Admissions Registered Nurse (RN) to join a clinician-owned hospice team. This full-time on-the-road role involves comprehensive admissions, patient/family assessments, and close collaboration with physicians and the interdisciplinary team to tailor plans of care.

Travel within service areas is required, and a valid RN license is essential. The position emphasizes compassionate end-of-life care, strong communication, and the ability to educate families about disease

Qualifications

• Active RN license is required.
• 1+ year of skilled nursing experience preferred.
• Hospice/admissions experience is preferred but not required.
• Willingness to travel within assigned service areas.
• Strong communication, organizational, and interpersonal skills.
• Valid driver's license, reliable transportation, and auto liability insurance.

Responsabilités

• Complete comprehensive hospice admissions and patient/family assessments.
• Ensure accurate documentation and appropriate care planning.
• Provide direct patient care as needed, including medications and treatments.
• Collaborate with physicians and the inter-disciplinary team to develop plans of care.
• Serve as liaison between patients, families, and healthcare providers.
• Educate patients and caregivers on disease progression, symptom management, and hospice services.
