Job summary

VITAS Healthcare is seeking an admissions nurse who will be the first face of hospice for patients and families. You will travel to meet with patients and families, educate them about hospice services, and assess end-of-life needs with empathy to determine if hospice is the right path.

• Current RN license is required.
• Reliable transportation is required.
• Experience in a physician’s office, hospital, long-term care or assisted living facility.

Responsibilities

• Travel to meet with patients and families.
• Educate patients and families about hospice care and the services offered by VITAS.
• Ask questions and listen with empathy as you learn about each patient’s end-of-life needs, wants and beliefs.
• Use this information to determine if hospice is the right path for the patient.
• Ensure a comfortable transition for patients who elect to receive hospice care with VITAS.
