Job summary

WesleyLife seeks an Admissions Nurse for its Hospice team to travel to client homes and facilities, conducting comprehensive assessments and initiating individualized care plans. The role involves coordinating orders, providing patient and family education, and supporting daily living needs in the home care setting.

The nurse will perform head-to-toe exams, HIS assessments, and adjust plans as patient status changes, working closely with physicians and caregivers to ensure high-quality hospice

Job description

As an Admissions Nurse for our Hospice team, you will:

• Traveling to CHLs and homes to see our hospice clients and their families
• Complete initial, resumption, follow up, a significant change of status and discharge assessments including comprehensive head to toe assessment, HIS assessment, and all associated assessments with admission or discharge
• Initiate an individualized plan of care, and update the plan of care for resumptions, significant change in status, and re-certifications.
• Use health assessment data to determine nursing diagnosis
• Obtain, implement, perform and maintain orders as received by physicians overseeing patient care services
• Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician
• Counsel the patient and family in meeting nursing and related needs
• Provide health care instructions to the patient as appropriate per assessment and plan of care
• Assist the patient with activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
