Job summary

Lower Cape Fear LifeCare seeks a compassionate Nurse Navigator to guide patients and families as they begin hospice services. You will perform initial assessments, coordinate care, and support patients in their homes or care facilities.

In this outpatient-adjacent role, you will collaborate with physicians, RNs, social workers, chaplains, and other IDT members to ensure smooth admissions and compliant documentation. The department operates 7 days per week with a 4x10 schedule.

Responsibilities

• Conduct initial assessments and plan of care during admission visits.
• Coordinate with IDT members to ensure seamless admissions into hospice care.
• Collaborate with external partners to organize admissions and referrals.
• Document eligibility for hospice based on clinical criteria (e.g., uncontrolled pain, symptom needs).
• Educate caregivers and families on end-of-life expectations, medications, and symptom management.
