Queen City Hospice in Southwest Ohio is seeking an RN for Hospice Admissions. You will guide patients and families through the start of their hospice journey, conducting pre-admission evaluations and initiating the admission process.

Qualifications

• Graduate of an accredited RN program.
• Active Registered Nurse license in the state of practice.
• Current CPR certification.
• Hospice experience preferred but not required.
• Strong clinical judgment and ability to work independently.
• Compassionate communication with patients and families.

Responsibilities

• Conduct pre-admission evaluations to determine eligibility and appropriateness of care.
• Educate patients and families about hospice services with compassion and clarity.
• Obtain informed consent and initiate the admission process.
• Perform comprehensive nursing assessments and develop individualized care plans.
• Deliver interventions that maximize comfort, dignity, and quality of life.
