• Graduate of an accredited nursing program with current licensure in the operating state.
• Hospice nursing experience preferred (minimum 1 year).
• Must be a licensed driver with a valid auto insurance.

Responsibilities

• Visit patients referred for hospice care in homes, hospitals, or facilities.
• Explain services, discuss goals, and obtain consents and documents.
• Perform comprehensive clinical assessments to determine hospice eligibility.
• Coordinate start of services and collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
