CompassionCare Hospice in Carson City, NV is seeking a passionate Hospice Admissions RN to join our team. Our RNs provide patient-centered care at a critical time in people’s lives, offering a supportive environment where your skills and empathy shine.

The role involves visiting patients in homes or facilities, explaining hospice services, collecting medical histories, and coordinating start of services. Requires RN license, hospice experience, and CPR certification.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care.
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
