Job summary

Advanced Home Health and Hospice in Carson City, NV is seeking a Hospice Admissions Nurse to manage the admission process for new patients, coordinate with the care team, and deliver compassionate nursing care. The role requires delivering patient- and family-centered support during the admission period.

The RN will perform assessments, documentations, and collaborate with physicians and social workers to ensure smooth transitions into hospice services, while maintaining regulatory compliance

Qualifications

• Valid RN license in Nevada.
• Minimum of 2 years RN experience.
• Strong assessment and critical thinking skills.
• Excellent interpersonal and communication skills.
• Knowledge of hospice regulations and documentation.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess incoming patients to gather medical history and identify care needs.
• Develop individualized care plans with the interdisciplinary team.
• Communicate with patients and families about the admissions process and services.
• Complete admission paperwork and obtain required documentation.
• Coordinate with social workers, chaplains, and volunteers for admissions.
• Administer prescribed medications and manage pain and symptoms.
• Monitor patient condition during admission and report changes to the team.
• Provide emotional support to patients and families during admission.
