Job summary

COMPASSION CARE HOSPICE is seeking a highly motivated Hospice Admissions RN to serve Carson City, NV. You will visit patients in homes, hospitals, or facilities, explain hospice services, obtain consent, and assess eligibility according to Medicare guidelines.

The role supports independent work with strong team collaboration, offering flexible scheduling and opportunities for growth in a growing company.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• CPR certification in states where required.

Responsibilities

• Visit patients (in homes, hospitals, nursing facilities, etc.) referred for hospice care.
• Explain hospice services, discuss goals and care with patient and families, obtain consents and legal documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine hospice eligibility based on Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.

As a Hospice Admissions RN You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
