Job summary

Chapters Health System is seeking an Admission RN to assess patients at their residence or in hospitals and initiate care within our system. The role involves ensuring a seamless transition to Care Navigation and representing the organization professionally.

The position requires an active RN license, at least one year of nursing experience (hospice or hospital preferred), and experience with EMR/EHR systems. Strong communication skills and the ability to work in a team are essential.

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Ability to effectively communicate in English (verbal and written); bi-lingual in Spanish preferred.
• Strong clinical assessment skills with background in emergency department or ICU.

Responsibilities

• Represent the Company professionally at all times through care delivered and/or services provided to all clients.
• Comply with all state, federal and local regulations, maintaining a strong position against fraud and abuse.
• Observe the Company's health, safety and security practices.
• Maintain patient confidentiality and sensitive information.
• Collaborate with Care Navigation and refer to all service lines as needed.
• Communicate with Care Navigation staff to ensure timely follow-up.
