Compassus in Wisconsin is seeking a Hospice Admission Registered Nurse (RN) to evaluate patient eligibility for hospice, inform families about services, and coordinate initial care with the care team. This role focuses on enabling patients to spend meaningful time with loved ones while delivering compassionate, patient-centered admission assessments.

Qualified candidates hold a current RN license in Wisconsin, with at least two years of nursing experience and CPR/BCLS certification.

Competenze

• RN license in the state you work.
• Two years of experience as a registered nurse; hospice experience preferred.
• Current CPR and BCLS certification.

Mansioni

• Enable patients to spend quality time with their loved ones and doing activities they enjoy.
• Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers.
• Ensure all patient’s initial needs are addressed.
• Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information.
